Instructions: Proofread and correct errors. Dear dr James I saw your patient in consultation this afternoon. She is an 84 year-old African-American female who presented to you with a history of shortness of breath and chest pain radiating to her left arm; She denied any other symptoms currently however she stated that last year she had a mild episode of chest pain that resolved without intervention. Her past medical history is basicly unremarkable. She has been in good health her lifelong. Both parents lived well unto their eighties. On physical examination, she appears well nourished and in no acute distress. Vidal signs was normal. Lungs showed a few scattered weezes. Heart revealed a regular heart rate and rythym. Abdomin was soft no masses were noted. The EKG taking yesterday showed questionable myocardial damage. Laboratory data was normal I told this nice lady that she had suffered a heart attack and would need to be hospitalized for a few days for evaluation and medication adjustment.
