In the spreadsheet is a list of companies that had a material weakness in internal controls from 2011 to the present. Two are required and the others are provided as a "backup". You can pick which of the ones assigned to you to use. The location of the internal control report is listed in the colored columns. Some of you will also see that the source document is a 10-kA. This means the 10-k had an amendment. Searching in EDGAR on the SEC database for the source file; then identify the date the file was submitted (also in one of the colored spreadsheet columns). If one of your companies assigned has more than two material weaknesses, then I would recommend using one with fewer.
This list is not complete with respect to all companies that had a material weakness during that period. I filtered the complete list.
