Immigrant cultures-practice infibulation

What happens when a client's belief system and that of the therapist are in significant conflict with each other? For example, although it is illegal in the US, some immigrant cultures still practice infibulation (surgically closing the labia in women to prevent intercourse). How would you work with a client from that culture who was seeking help to overcome her fear of the procedure her parents were demanding of her. In other words, she believed going through with the procedure was the morally right thing to do; she was just afraid of it,so was wanting you to help her through her fear.
