The patient is 95 year old male with a history of type 2 diabetes mellitus, lung cancer with mets to brain in remission, hypertension, venous insufficiency, hard of hearing, depression, falling, and abnormalities of gait and mobility who has just moved into Grand Living at Naples Assisted Living Facility. He is alert and oriented, and hemodynamically stable. Denied having chest pain, palpitation, shortness of breath, dizziness, headaches, blurred vision, chills, night sweats, coughing, sore throat, running nose, nasal congestion, stomach pain, nausea/vomiting, or loose stools, with the exception of bilateral lower extremity edema, unsteady gait requiring a walker, and will need HHC referral for physical therapy to aid with his weakness, bilateral upper extremities wounds and will need HHC referral to provide wound care. He would also benefit from occupational therapy to help with activities of daily living (ADLs) and restore joint mobility. He also need continued chronic disease management, routine cardiovascular and blood pressure monitoring, the continuation of his current medications, and a weekly follow-up visit to review his general health status.
