Your client is Jim, a 36-year-old car mechanic. Jim was diagnosed four months ago with a "Mood Disorder-Mania with Psychotic Features." His delusions are the grandiose type. Jim's current presentation is consistent with mania. The psychiatrist prescribed Prozac (Fluoxetine), 40 mg per day and Risperdal. After 2 months, Jim was experiencing moderate relief from his symptoms, but is now bothered by racing thoughts and an inner restlessness-to be in constant motion. John wonders if he should consult again with his psychiatrist and if there is anything that can be done. He is not sure if his problem can be resolved.
Clinical Social Workers possess a range of roles such as consultant, counselor, monitor, advocate educator, and researcher with clients who take psychotropic medication. As a means of preparing for such interventions, the social worker should reflect on and be able to articulate responses to the following questions:
I. Educate John regarding extrapyramidal side effects (EPS) in relation to his medication. Be specific.
II. Research and explain two medication strategies that may be used by the prescriber to correct EPS.
III. How would you counsel John on the management and reporting side effects to the prescriber?
IV. Explain to John your role as a monitor and consultant with the prescriber.
V. As an advocate educator, discuss with John the desired outcome of his medications.
