Consider the following as you complete the assignment.
1. How would you communicate with your patient and what interview techniques would you utilize for your patient to build a health history? How would you formulate your questions based on the patient's social health factors? What risk assessment tool would you utilize with your patient and what questions would you ask to assess health risks for your patient?
2. Identify any potential health risks based on the patient's age, gender, ethnicity or environmental surroundings that you would consider important to the patient's health.
3. Select one of the risk assessment instruments presented in chapter 1 or chapter 5 of the Seidel's guide to physical examination text or another tool related to your particular patient.
4. Develop at least five targeted questions that you would ask your patient to assess health risks and begin building a health history.
Post a summary of the interview and a description of the interview techniques that you would use with your patient. Explain why you would use these techniques. Identify the risk assessment tool that you selected and why it is the appropriate tool for your patient. Provide at least five targeted questions that you would ask the patient.
Format your assignment according to the following formatting requirements:
1. The answer should be typed, double spaced, using Times New Roman font (size 12), with one-inch margins on all sides.
