Problem

Where do you see a need or gap in the community?

Refer to the textbook Health and Health Care Delivery in Canada (Thompson)

Students will develop a community-based health care program of their choice in an area of personal interest (Ex. outreach, campaign, recommendations, home support). Think of a specific target group (Ex. homeless, immigrants, seniors, students, mothers).

The following steps should be taken as an outcome-based checklist:

A. Why are you creating this program? What is its role?

B. Who is the target/audience group? Who in the community are you trying to help?

C. What is the service/activity of the program?

D. What will be the benefit or result/outcome of your service/activity? (eg. Change in knowledge, teach behaviour, provide resources, support feelings etc...)

E. How will you see and measure the success of your program? (increase, decrease, maintain through survey, testing, goal attainment)

F. What is the vision of your program? If you could wave a magic wand and all of your program goals were met, what change in social condition would occur?

G. Consider sources of funding and other organizations that can work in tandem with yours. (How will you get money to run the program and what other organizations will work with you).

Based on your learning of the current healthcare system, you should be able to describe what resources and information you may need for specific actions so that you may deliver to your participants optimal information and demonstrate your goal to achieve change (short-term outcome), to address community problems (mid-term outcome) which will, in turn, solve a larger community need (long-term outcome).
