o Type of provider being credentialed
o Steps they will go thru to be credentialed
o Educational background needed
o Does this provider have an NPI number?
o State the provider will work in
o Is there a standard application?
o Is an onsite visit needed; why or why not
o Who gets credentialed (list at least 5 specialties)
o Are all facilities credentialed?
