Visual Works: take a screenshot or picture and insert it on a slide
Visual/Dance/Audio: Include an audio file or provide a link to the YouTube/Vimeo/Soundcloud
Text work: Provide a screenshot of the cover page or a link to the work
On the same slide, include a description of your work, year of authorship, and your name (which is a copyright notice)

PA Form used for film, movies, choreographical works, music that will have a music video (NOTE musical works are encouraged to use this form because it is likely you will create a music/YouTube video).
