At a meeting about 6 weeks ago held between Service Provider 1 and 2 and you, it was discussed that establishing a relationship with a third organisation might further support Lin's needs. After the discussion, it was agreed that working with local Vietnamese community members might help Lin and her Mum, Mai. Lin has expressed interest in learning more about Vietnamese cuisine and possibly establishing a career in either food preparation or growing vegetables used in Vietnamese dishes.
Discussion with Lin also identified that Mai would benefit from this, as she is losing her ability to converse in English as her dementia progresses. Lin feels that if Mai were to expand her social network with fellow Vietnamese speakers, it would help her to maintain her independence. Lin feels that it would also help her as she would feel less anxiety about her Mum's wellbeing if she knew she could meet and talk with like-minded members of the Vietnamese community.
To this end, you established contact with the Local Vietnamese Community, an informal social support network for members of the Vietnamese community. The group meets weekly to tend its community garden, share food and play card games. You met with Duc Pham, the group's contact person, who explained that this is an informal organisation and receives no government funding. The only support they receive is access to the local council community garden and centre.
At the meeting, it was agreed that Duc would contact Lin and Mai and invite them to come along to the community garden. He would ensure that they were introduced and given the opportunity to explore the activities. - She feels she is making good progress towards her first 2 goals and is feeling much calmer and in control of her emotions. She has been socialising more and feels this is a direct result of the work she has been doing with Service Providers 1 and 2.
- Lin and Mai had an initial meeting with Duc from the Local Vietnamese Community, and both were really excited about the opportunities discussed at the meeting. Mai has been attending weekly, and Lin has noticed that this makes her Mum happy. Lin said she had been down there a few times after making arrangements with Duc to participate in food prep or gardening, but he either had not been there at the agreed time or had been too busy to help her out. She is feeling a bit frustrated and let down.

- Service Provider 1 has been meeting with Lin on a weekly basis and feels she is making good progress. They document these discussions and share notes with you and Service Provider 2. They note that they are receiving update notes from Service Provider 2, but these are often some weeks old when they are received and contain out-of-date information.
- Service Provider 2 also feels Lin is progressing but perhaps not at the anticipated rate. They have had to cancel some meetings with Lin and you due to staff shortages. This staff shortage has also resulted in the organisation falling behind in its administrative tasks.
Representatives of both Service Providers recently met at a local area network meeting. This meeting allowed the representatives to meet face-to-face for the first time. During the discussion, it became apparent that they have many clients that could benefit from each other's services
