How population is distrubuted

Problem

You'll read that those who do live in rural and remote areas are more likely to smoke and to have a higher incidence of other health and behavioural issues / factors. We also know that the distribution of health services reflects how population is distrubuted - so how do we reconcile this issue? Are those who live in isolated areas are more at risk of health issues yet unable to access the same level of health care as metropolitan populations?
