How has your organization done to reduce communication barriers?

Is there some policies and procedures a hospital can put into place to help reduce the errors from happening? I agree that hospital should pay the patients hospital bill, but I also think they need to cover any medical bills in the future that could arise do to whatever mistake was made. I also think hospital should cover any loss wages that might have occurred. What do you think?
