Traditionally, performance reviews are completed using preset performance appraisal forms or templates. An appraisal form should have a detailed framework for measuring performance. Whether you are an employee, HR representative, or manager, you will have to conduct and/or review performance appraisals.

Draft up a performance appraisal template that you believe would benefit the organization for which you currently work or have previously worked. Prior to beginning work on this exercise, please review Chapter 6: Performance Appraisal: Measurement, Assessment, and Management, and read the webpage Viewpoint: How to Redesign Your Performance Appraisal Template. Additionally, review several performance appraisal templates available on the Internet.

Part I

Use a performance appraisal template that you feel meets the needs of your current or previous position and organization.

1) Include in your appraisal a rating scale and five competencies you would recommend the organization to evaluate staff on. At a minimum, include a rating scale guideline (i.e., description of what each rating is composed of).

2) Attach your performance appraisal template.

1) How does your selections contribute to effective employee performance management, training, and development programs.

2) How does human resource information system (HRIS) could be used to support an effective performance management training and development program.
