#1. Hello everyone, after some deeper research, how does data capture help with quality management and reporting? How does it assist with continued accreditation and government/insurance reimbursement for continued treatment? Does it show accountability? If so to whom? How does data capture allow for improvement? And finally, are there any tools available to assist with data tallies to report on?

#2. The healthcare organization that I am most familiar with is the VA Medical Center. Information capture is very important not only to the VA but most medical centers and healthcare organizations because its what some may refer to as the "bread and butter" of their profit and outcomes; meaning that this is where patients will register and gain their status in the medical center and this is also where insurance and payment information is taken or "captured".

During my time at the VA information capture what a frequent topic of discussion in leadership board meetings. This topic there was mainly focused on the most efficient way to capture a person's information that was already in the system but had changed addresses, phone numbers, and other points of contact. This was such a heavy topic at the VA because of our information capture system design; the way the program was designed certain fields were unavailable unless it was a first time entry. This was an issue because when a returning patient came in to registration there was never a field or prompted question that asked about any changes in phone numbers or addresses and many patients forget to mention it. This become quite the problems when employees are unable to get in touch with patients for follow up appointments, test results, and other instances.

Information captures systems for many health organizations serve as data collection tools. For example, when a patient in the Emergency Department is with the triage nurse and she or he is asking question about their medical history or their environment. That will all be accounted for and later used for evaluation purposes. Proper information capture techniques can also lead to better patient outcomes long term because it lets hospital personnel and other officials know what their patients are suffering from and in some instances they are able to figure out why and how to make patients better through information capture practices.
