Discussion Post: Quality Improvement & Patient Safety
I. Transparent Health: Lewis Blackman Story
In this first of four discussion boards, you will watch the video titled, "Transparent Health - Lewis Blackman Story. In this video, two issues in health care will be addressed: (i) prevention of medical errors and (ii) how we the care givers respond when our care has caused harm to one of our patients.

Task:
After viewing this video, you and your nurse colleagues are to discuss the following:
i. How did the health care system fail Lewis and his family?
ii. What can we, as nurse leaders, learn from the Lewis Blackman Story?
iii. As a health care leader, describe strategies/approaches to effectively address medical errors as described in this story about Lewis Blackman, e.g. teamwork and communication.
II. Reflective Journal
Separate groups: Group A
The Reflective Journal is a place to write short reflections that are posed during the course in Lessons and elsewhere. These are private between student and instructor and allow a non-formatted space for reflecting about your weekly learning and help you realize how you have changed based on the learning you have accomplished during the course.
Make sure to enter a "Heading" for each reflection that your are entering to keep them separate.
The response must include a reference list. One-inch margins, double-space, Using Times New Roman 12 pnt font and APA style of writing and citations.
