It is indeed difficult for me to come to the realization that this course is over. It started off for me like a new adventure, and suddenly, it is over. In this adventure, I experienced the "aha" moment when I conducted the concept analysis project on Meleis' transition theory. The step-by-step analysis transition concept practice sculptured my understanding of this theory. In the application of nursing theory assignment, I applied transition theory to solve readmission issue, and through this exercise, I gained a clear insight that connection between nursing theory and practice.
