Problem: Condense this Clinician first saw the client for an initial session on 4/2/26 then another session4/22/26 we were able to develop a treatment plan during these session that focusing on anxiety related issues managing anxiety identify and express their sources of anxiety, fears, and concerns, and will learn techniques to self-soothe when feeling anxious. following that client no showed the next session 5/4/26 client has noshowed first session schedule with this cinicina on 3/25/26 before making it to 4/2/26 client was called by open acess staff to schedule a ECM on 5/11/26 to discuss barriers that are present and see how to suport client mother schedule 5/12/26 no show then schedule 5/21/26 no show as well. based on client no shows and engagemnt and attendenacy policy client is currently being discharfe from practice. as far as vocational history or subtance abuse client is too young to be access for that client and her family were currently facing finaccial struggles and housing instablity.
