Problem: Write a follow up psychiatric note, chief complaint, current Meds, HPI, formulation, recommendation treatment plan for a 34 years old Female with diagnosis of Obsessive-compulsive disorder, Attention-deficit hyperactivity disorder, predominantly inattentive type Chief complaint: Current Meds: None. Past HPI: He said he struggled with overstimulation, restlessness and anxiety from the Methlphenidate 20mg ER. He said his mind is currently always thinking of so many things. He said initially he was on 10mg daily with a previous provider and later on his dose was increased to 20mg with another provider. He Said Methylphenidate helped with his concentration and his work productivity improved then too. He said he works for Olympic College in Bremerton Washington State. He said he then stayed a month without his medication and when he got finally refilled his 20mg, that is when he struggled with overanxiety and overstimulation.
