"If an operational entity is in obvious trouble .... regressive in format & superficial in production, then a Root Cause Analysis (RCA) is warranted to identify, isolate, quench & uproot the cause(s) of shamble. Immediately after, a Corrective Action (CA) must be issued in order not to allow the recurrences of failure.
Otherwise, ignoring the detrimental & factual causes on the ground will naturally revert to the status quo, and more personnel will have subscribed to sustaining the chaos.
