Miss Nesbit is being discharged from a healthcare facility. The discharge summary is created to provide information and a plan of care for the patient as well as for future healthcare providers. Discharge summaries should include the reason why the patient was initially seen or admitted to the facility, what was found, the procedures or treatments that were administered to the patient, the patient's condition on discharge, instructions for the patient and family, and the attending physician's signature.
Read Miss Nesbit's EHR Discharge Summary. Then, complete the following:
a) Explain to Miss Nesbit and her family, in your own words (using layman's terms), what each section (subjective, objective, assessment, and plan) of the Discharge Summary means.
b) To do this, you will have to look up all of the bold terms and abbreviations in the Discharge Summary.
