Contraindications of Exercise and Informed Consent
Before setting up an exercise program for an individual, it is important that you complete a health history, explain the Informed Consent form, and assess contraindications of exercise of the client. These items should be completed by a qualified health care professional, such as you. As you learned in One, individuals within a high risk category with known disease should be seen by a physician prior to exercise.
Assume you are working with an individual who has a moderate risk of experiencing health-related problems during exercise. Review the sample Informed Consent form and then complete the below:
