Miss T is with type uncontrolled diabetes who is has been previously seen in the community by home care. Today she is being admitted to your unit with a large wound on her foot that is not healing. The physicians are worried her wound will infect the bone and lead to amputation if it worsens. She has also been coughing up blood tinged sputum for the last month. She reports being tired and waking drenched in sweat with fevers every few days. The attending physicians orders Miss T to be on isolation for suspected tuberculous and daily dressing changes. You enter Miss T's room to change her dressing in your PPE. Miss T reports being very fatigued and you asses that she cannot safely get out of bed to transfer. You decide to leave Miss T in bed to change her dressing, and deliver her medications. Miss T asks you why you are checking her medication three times and states "they are the same as yesterday". Finally Miss T asks if you could cut her toe nails before exiting the room as she is unable to do so herself.
