Part A: Dashboard

Using Microsoft Excel or PowerPoint, create a quality dashboard based on the Community General Hospital Case Study. Your dashboard must include 6-8 measures. Use mock data to represent the measures you have chosen.

Part B: Written Summary

To accompany your dashboard, write a 2 to 3 pages paper in which you do the following:

1) Identify the 6 to 8 quality measures you have chosen for your dashboard.

2) Explain why these measures are important to the organization.

3) Analyze how the Triple Aim/Quadruple Aim is represented in your chosen measures.

4) Explain how you displayed the measures. Justify your choice of display.

5) Provide a strategy for communicating the dashboard throughout the organization.

6) Explain how the dashboard could be used as a leadership tool to improve patient outcomes.

Format your assignment according to the following formatting requirements:

(1) The answer should be typed, double spaced, using Times New Roman font (size 12), with one-inch margins on all sides.

(2) The response also includes a cover page containing the title of the assignment, the student's name, the course title, and the date. The cover page is not included in the required page length.

(3) Also include a reference page. The Citations and references should follow APA format. The reference page is not included in the required page length.
