Expanding interprofessional teams to include pharmacists

Problem: Expanding interprofessional teams to include pharmacists, social workers, and community health workers can address social determinants of health and support predictive analytics to find vulnerable patients (Shiyanbola et al., 2022). Innovative care models such as Patient-Centered Medical Homes (PCMHs) and Accountable Care Organizations (ACOs) further promote coordinated, patient-centered, and cost-effective care (Ginting et al., 2022; Holm et al., 2024). Future evaluations should track A1C levels, medication adherence, hospitalizations, and emergency visits using EHRs, wearables, and patient-reported data. Tools like SPSS and NVivo can support mixed-methods analysis. By challenging assumptions and applying scalable, tech-enabled, team-based approaches, the DSMES intervention can expand its impact and sustainability across diverse healthcare settings.
