Case Manager
Primary Role: Coordinate services, documentation, and communication with families and partners.
Key Responsibilities:
- Assist with referrals, scheduling, and collateral contacts.
- Support discharge planning.
-Work closely with clinical team to implement treatment goals within day to day functioning
- Maintain documentation and reporting compliance, complete psychoeducational groups and weekly case management notes
-Work with nursing to coordinate and facilitate medical appointments as necessary
