Problem: Help me write a HPI for an Initial Psych intake based on these information: Patient comes in for an initial evaluation for medication management of past psych history of ADHD, Bipolar I, GAD, insomnia, substance abuse. He have a long hx of Bipolar diagnosed at eight years old. He have a long history of sexual, physical, and mental abuse from family and ex partner. Patient is currently divorced living with room mate, is gay and does have a current partner at this time. Pt is going to therapy, has been seeing current therapist since October. Pt have a long history of being on psychomedication. Only currently on Buspar 15mg BID. Has been on medication since 2020 after hospitalization for Sucide ideation. works for Amazon. Will be starting school for computer programming. Move to new place in October. Stated he has been feeling better and looking forward to his day. He denies hallucinations, delusions. Denies active suicidal ideation, plan, or intent. Hx of cocain, meth, marijuana use, and excessive use of caffeine. Hx of nicotine use 1-2 pack a day, stopped in June 2023 and switch to vaping until October 2025. Last 6 months, still feel anxious and overthink. Reported anxiety comes off as angry, feels like someone is squeezing on his chest. Current appetite has been good, happy with current weight at the moment.
