Evaluated for an accidental fall


Problem: A 65-year-old male originally from Greece is evaluated for an accidental fall. He tripped on an unnoticed obstacle at a parking lot. He is complaining of mild pain over the right hip, where he landed. Images of his head and right hip are obtained. No fractures are evident; however, the CT scan of the head shows thickening and sclerosis of the entire skull (see Figure). Figure. The patient denies any preceding symptoms. The wife at the bedside reveals the patient has been losing hearing for the last year or so. The patient also likes to wear hats and apparently is requiring larger sizes as compared with before. What is the likely diagnosis in this patient?
