Assessment Interview between Case Manager and Client

The assessment interview process involves the meeting of a client with the case manager who gathers information to address the client's direct and immediate needs to encourage his or her retention and engagement in the services. This assessment interviews also help to screen clients and know the extent of case management services that they will need. This also helps decide the type or model of case management most efficient and appropriate to meet a client's need and at the same time assess the client's readiness or willingness to engage in the case management services. The known case management models are two; supportive case management model where brief intake or assessment is the only mechanism for assessing the needs of the client and documentation from this model gives the basis for developing a brief service plan. There is the second model known as the comprehensive case management model that allows initiation and start of the case management activities until when a comprehensive assessment can be finished.

In all phases and activities case managers must assure that the client's confidentiality and privacy is maintained through the whole process of case management. Aiming at a specific population like those affected with HIV, there are general standards on how case assessment should be conducted. Generally, key information concerning not only the client, but his or her family, caregivers and documented to determine client eligibility to be enrolled. This makes it possible to know client's need for ongoing case management services and the appropriate level for the case management service.

There are criteria that need to be followed when doing an assessment interview between a case manager and a client. The case manager needs to be able to put the client at ease so information can be able to be attained with efficiency. This requires a strength based approach where the case manager is able to balance his questioning from the positive perspectives giving confidence to the said client. Active listening is another skill that is needed during the assessment interview. To get and put down the right information from the client, the assessment manager has to have his/her ears ready to listen as the interview proceeds. This gives way to questioning skills and responding skills. It is an assessment interview and communication being a two way, questions will flow from both parties giving way for time to respond.

One important factor that will be tested is the case manager ability to understand a client during the assessment period. This requires patient and his/her ability to have a structured step by step approach. If need be clients information has to be shared to another party it can only be done with the knowledge and approval of the client. At the end of the assessment period the case manager should be able to know if the client will be in need of more assessment interview and give specific directives in accordance to the result of the assessment.
