Problem: A 65-year-old woman with worsening dyspnea, cough, and sputum production over several days is admitted to the medical unit with COPD exacerbation. She has a history of emphysema from cigarette smoking. She has been taking tiotropium and Advair routinely for years. She is allergic to penicillin.
