Problem
Mental health Case study: Liam was admitted to the mental health unit with a medical diagnosis of depression and having suicide ideation. He has been responded well to treatment and the mental health team are now preparing for his discharge. He will be transferred to the community mental health team for his care and medications once discharged. As an EN and his primary nurse, will run the discharge planning, in preparation for his return to home and community support.
(Liam: Liam is the client with a medical diagnosis of depression with suicide ideation. He does not like to talk about his depression.)
A. Discuss with Liam and solicit feedback from him on at least one (1) therapeutic intervention that was introduced while he was an inpatient.
(Suggest a genuine form of therapy which was introduced, and seek feedback from Liam with relevant justification as to the outcome of it)
B. Discuss with Liam the outcome of two relevant nursing interventions which were introduced while he was an inpatient in the unit.
(At least two nursing interventions and support that would be appropriate based on a client who is suffering from depression, enhanced by drug use.)
C. Discuss at least one form of medication with Liam and confirm he understands the use, administration method and possible effects of it (including known specific adverse effects).
(Discuss with Liam one form of medication that could be used, and specify its name, benefits and potential side effects.)
D. Advise Liam about at least two community resources and opportunities available, which are relevant to his situation.
(Mention a minimum of two services outside the hospital that Liam could access that may help or work with him to improve his condition. These should be real services available in Melbourne Australia)
E. Work in a manner that reflects respect for the person's dignity and uniqueness, and use strategies to empower the person to contribute to their own plan of care.
