A3 form where we state the issue, background, current condition, problem analysis, target condition, countermeasures, implementation plan cost, test, and follow up
It not a certaing length but the questions must be answered well. Its a healthcare project where we must think of a process that can be improved. I had to do it on missing inhalers in hospital.
Patients MDI are being misplaced while their stay at the hospital. What are the root causes and target condition, countermeasure, implantation plan to fix it. Cost benefit/waste recognition. Test and follow up to make sure it works.
