On June 2, 1984, a pesticide plant owned by Union Carbide India Limited (UCIL) leaked large amounts of methyl isocyanate gas into the atmosphere. Considered by many to be the world's worst industrial accident, the Bhopal gas disaster resulted in the prompt deaths of approximately 4,000 people.
Based on the MORT philosophy do the following:
1. Describe the MORT analysis procedure by discussing some of the causal factors that allowed the accident to occur and discuss the MORT process.
2. What may be some advantages or disadvantages in using a MORT process?
