Post Assignment Community Health Flyer OR Social Media Post Students will be responsible for developing a community health flyer OR social media post that will be used to outreach your pilot program. Remember to think of your target group while creating this flyer OR Social Media post as you want to ensure that it is representing them. Treat this assignment as if you were going to hand this flyer and post this on social media to members of your selected target population as a way to recruit for your health education strategy. The following information should be included on your flyer OR social media post:
1. Title of your pilot program
2. Program Goal
3. Target Population ( Men/Women, Age group, Race/Nationality, etc.)
4. National and Local statistics on your selected chronic condition (2 should be local, 1 should be national).
5. 1 behavioral and 1 Non-behavioral factor that contribute to the health problem you have chosen
6. Discuss 2 preventive measures that help decrease this chronic condition (ie. Medications, exercise, etc.)
7. Location and who to contact for more information
8. Incentive
