Q1. Identify at least two community-based groups to assist in the project. Be as specific as possible, i.e. if including the local government; include which division of the government. It is ok to include a national interest group; however you must identify a local chapter of that group or other explanation as to how that group is connected to the local community.
Q2. Identify at least one local or state political leader who can be approached to champion the project. Describe the rationale to include this leader, such as the leader's current work/legislative or policy interests that would indicate the person would support the project.
Q3. Develop one short term goal and one long term goal that serves as the plan for addressing the priority population diagnosis. The goals must be written in the SMART format.
