Problem:
A.D. is a 38-year-old African American male who came to the clinic with a chief complaint of chronic right shoulder pain, which increased in severity about three weeks ago after participating in Army Annual Training at Camp Shelby. Patient says he had recurrent episodes of pain in the right shoulder since 2025, but due to his occupation as a soldier, the problem has worsened with repetitive movements and overuse. The pain occurs in the right shoulder, and it is constant, aching, tight, deep, sharp, stabbing, and sometimes gnawing. Today it is rated as 6/10, but in its worst state he rates it 9/10. Associated complaints include soreness, difficulty moving the joint, inability to sleep, and numbness and tingling that radiate to the upper extremities. Pain is triggered by lifting, reaching, or standing for a long time but is alleviated by rest and changes in position. Acetaminophen, ibuprofen and methocarbamol were used before with little effect. It affects his work performance, physical activity, and his ability to play with his son.
