Write a follow up psychiatric note, chief complaint, current Meds, HPI, formulation, recommendation treatment plan for a 63 years old male with diagnosis of Obsessive-compulsive disorder, unspecified Current Meds: No current Meds and does not want any psychiatric medications during this appointment. Past Medication: Venlafaxine 75mg ( was not taking the medications because of the side effects) and Sertraline 50mg (stopped due to Side effects) HPI: He was well groomed and casually dressed for the weather. he said he has not yet travelled as planned and his mental health is not in good shape. He said he wants to discuss about a place he can stay for recover his mental health because he struggles to interact with people in the shelter because of his diagnosis. He said he wants too some problems in creating relationship with people because he is always aware of his surrounding and needs a quiet place to live. He said he is currently living in a shelter and needs a better place because he wants things done in a specific way but living in the shelter makes things worse because people do things anyhow and this affects him a lot. HE Said he needs a letter stating his medication and his conditions so that he can show his sister and let the sister remove him from the shelter to her house .
