Problem:
IP is 35-year-old Caucasian female with hx of unspecified schizophrenia spectrum and other psychotic d/o; ADHD, combined presentation; opioid use d/o. She was admitted to CIW MHCB from CCWF EOP on 02/18/26 for SIDMI. Per MH Consult Inpatient, "I/P appears to suffer from either organic MI or substance induced psychosis and evidenced limited insight and resistance to medications. She is on Sw1:1 Q15 mins for safety, she is withdrawn, with flat affect and poorly engaging with staff. She reports feeling depressed, anxious Pt Observation- SW1:1 She reports that she's not doing well but won't elaborate. She's withdrawn, presents lethargic, depressed, anxious, and remains with limited engagement. She said that she's having significant difficulty managing depressed, denies any sleep issues, appetite is good, ate 100% breakfast this morning. IP is authorized to have reading material and hygiene products given but has no yard privilege out of precaution for safety. IP enc to verbalize feelings and needs and to utilize coping skills stated likes to read and a book was offered but went straight back to sleep. Will cont to monitor for safety. Need Assignment Help?
