Problem: Write a follow up psychiatric note, chief complaint, current Meds, HPI, formulation, recommendation treatment plan for a 17 years old Female with diagnosis of Major depressive disorder, Recurrent episode, Mild Chief complaint: Current Meds: None. Past Fluoxetine 10mg daily. (Patient did not continue this prescription because Provider was far from her and she had transportation issues. HPI: During the last appointment we discussed about decision of starting Fluoxetine 10mg daily, patient had to research on this medication and notify provider on her decision today. Patient said he has decided to start this medication. She was on it before and it did not have any side effects. Patient has been notified of the black box warning of Fluoxetine and she verbalized understanding. Patient verbalized poor sleep, and recently less eating because her appetite has reduced. She describes her mood as having a depressive episode stating today it has been hard to even get out of bed and do her daily necessities. Denies SI and HI . Follow up in two weeks.
