Problem: The purpose of this capstone project was to address the critical need for accessible, effective diabetes self-management education among adults with type 2 diabetes mellitus (T2DM) living in rural and underserved communities. These populations face significantly higher rates of poor glycemic control, diabetes-related complications, and hospitalizations due to barriers such as healthcare workforce shortages, geographic isolation, and limited access to Diabetes Self-Management Education and Support (DSMES). In response, this project proposed a structured, nurse-led, telehealth-enabled DSMES intervention, adapted for rural settings, to enhance patient engagement, promote health equity, and improve glycemic outcomes. The project used a quality improvement approach grounded in Plan-Do-Study-Act (PDSA) cycles to implement and refine the rural-adapted Diabetes One-Day (R-D1D) session. This interprofessional, culturally tailored program aimed to overcome common access barriers by delivering DSMES through telehealth within a community-based rural primary care setting. The intervention focused on individualized goal setting, education, and support, targeting adults aged 45 and older with uncontrolled T2DM. Preliminary findings indicate that structured DSMES programs can significantly reduce HbA1c levels and improve self-management behaviors in rural populations.
