Youth JV was picked up from school for dismissal, upon dismissal youth was brought over to the administration to await for staff arrival. Youth began to antagonize a housemate regarding an incident that transpired the day before. Youth was redirected multiple times by staff L. Severe, youth JV continued to veralize threats of physical harm upon returning back to the home. This writer attempted to speak to JV about her threats of physical harm to housemate in which she continued to verbalize that she didn't care about the other youth's feelings. Both youth were offered a mediation, JV refused she continued to verbalize threats. Peer was removed from the situation as JV began to escalate, she began to pace back and forth in the office. JV behaviors escalated quickly in which she attempted to walk pass the desk throwing and kicking items, staff attempted to deescalate youth using de-escalation techniques, but were unsuccessful. JV grabbed a chair located in the office and attempted to throw the chair. JV was immediately placed in an upper torso restraint by staff L. Severe. JV began to kick aggressively resulting in kicking the front desk were the admin staff was sitting almost causing entire desk to flip onto staff. JV was immediately placed in a multiple person supine restraint by staff L. Severe and D. Acevedo. While in restraint JV continued to verbalize threats to both peer and staff saying things like "Bitch I'm going to kill you when we get back to Shine", "Bitch you're a stupid whore", she continued to request that staff release her or she was going to kill us all. JV attempted to spit multiple times while in restraint. JV began to kick her legs staff C. Jones was requested to join restraint and gain control of her legs. JV was able to calm down and reset after about 8 minutes.  While in restraint staff continued to console and deescalate youth by validating her feelings and encouraging her to use her coping strategies.  After youth was released from restraint, youth verbalized being calm and began to express her by advising staff of the reasons why she was frustrated. JV verbalized being upset with housemate while also being upset that her mother has multiple children at home that she cares for but feels as though her mother doesn't feel the same about her. Youth was encouraged to speak to her therapist further about her feelings of resentment towards her mother, youth was also advised to work with her therapist to identify coping strategies that she can utilize in moments where she is frustrated. Youth then stated that when she is this upset her mother helps to calm her down, contradicting what she said moments before. Despite this staff continued to encourage youth to work through her feelings in that moment.

Youth then became preoccupied with wanting to call her CMO to pick her up and sign her out of the program. It was explained to youth that CMO workers don't have the authority to sign a minor out a program to take them home. JV quickly switched to wanting to contact her mom to come and sign her out as she expressed feelings of not wanting to be in the program. Again staff encouraged her to work with her therapist through family therapy and individual sessions to identify goals towards making her way back home. Youth then wanted to contact her mother, she was advised that she should fully calm down before calling her mother. And she continued to request to contact her mother as that would be the only way she would calm down. Around 3:18 staff decided to allow youth to call her mother as she continued to state that she would not calm down until she spoke to her. JV continued to appear agitated and frustrated throughout the conversation, therefore the call was allowed. JV's mother answered the call asking JV how her day was going, JV immediately began to ask her mother to pick her up. Jv's mother tone and demeanor shifted quickly as did JV, staff advised JV to end the call and try again later upon returning the home. JV's mother began to verbalize to JV that she needed to be more responsible and take accountability for her actions, JV became extremely irate showing the inability to maintain positive communication. JV began to threaten her mother making comments like "I should have killed you and your kids when I had the chance" "I fucking hate you, I hope you all die", JV then pulled the phone off of the desk in rage. Staff again made multiple attempts to verbally deescalate youth, while you ignored all attempts. Youth began to pace around the office yelling and screaming. Youth attempted to leave the building in which staff blocked every attempt while encouraging youth to utilize deep breathing techniques to self calm. Youth ignored all attempts, she stated that walking around would calm her down, she was allowed to pace the office. Staff advised any additional attempts to hit others or damage property she would be placed in a physical hold to keep herself safe. Youth approached the printer and appeared to look for items to throw or use to cause harm. Youth then approached the office window as staff attempted to guide her away from the window using a bicep assist youth punch the window causing glass to shatter all over. Youth had visible blood on her fingertips staff immediately placed her in a multiple person upper torso restraint, due to youth continuing to attempt to fight and spit at staff restraint was transitioned to a seated upper torso into a multi-person supine restraint. While in the supping restraint youth began screaming stating her hair was being pulled, staff attempted to reposition youth as well as gain control of the youth to eliminate any chances of additional harm. Youth was in a supine restraint between 7-9 minutes. Youth was able to calm herself down by yelling and screaming, youth was slowly released and immediately began to cry, JV began to cry uncontrollable and was consoled by this writer, staff provided immediate first aid to youth's hand and once deemed safe to transition to nursing, youth was evaluated and assessed by on campus nursing staff Samuel H.  Need Assignment Help?
