After reviewing the Assessment Assignment I want to be sure that everyone is clear on the differences between delusions, illusions, and hallucinations as it is quite important to clearly understand the differences between them. For the discussion topic this week, I would like to review these concepts. For the posting this week please write a brief description of each, and include a simple example and nursing intervention for each of these three.
