· Identify a client who may be ready to complete therapy.

The Assignment

· Overview of treatment process

· Goal status (goals met, partially met, unmet)

· Treatment limitations (if any)

· Remaining difficulties and/or concerns

· Recommendations

· Follow-up plan

· Instructions for future contact
