DJ is a 45 year old diabetic and hypertensive for 12 years. He has Stage 2 kidney disease.
1. Develop and explain to him a treatment plan for this condition.
2. What are the expectations of his lab work at this point to have this diagnosis.
3. Provide an overview of Stage 2 kidney disease and all that is pertinent for this patient to managing his condition from a provider and a patient self-management standpoint.
