Develop an interview questionnaire to be used in a family-focused functional health assessment. The questionnaire must include three open-ended, family-focused questions to assess functional health patterns for each of the following:
• Values/Health Perception
• Nutrition
• Sleep/Rest
• Elimination
• Activity/Exercise
• Cognitive
• Sensory-Perception
• Self-Perception
• Role Relationship
• Sexuality
• Coping.
The response should include a reference list. Double-space, using Times New Roman 12 pnt font, one-inch margins, and APA style of writing and citations.
