Problem Statement
Develop a logical database design (relational schema) for the conceptual data model shown at the end of this document. All relations in your logical database design should be normalized to the third Normal form (3NF).
Submission
1. A cover page with the following information:
- IDS 410 - Business Database Technology
- Fall 2015
- Individual Assignment #2 (Database Design)
- Logical Database Design for Lake Shore Hospital
- Your name and your last 4-digit of your University ID Number (UIN)
2. A logical database design or relational schema for Lakeshore Hospital. Use arrows to show referential integrity constraints. You don't have to show the functional dependencies among the attributes within a relation (table), but you have to make sure that all your relations are normalized to the 3NF. You may use a computer-based graphic tool to draw your diagrams such as VISIO or Gliffy (visit https://www.gliffy.com)
