This is indeed a challenging question. In healthcare, there will always be ongoing changes aimed at improving outcomes. The understanding that this dissertation will be difficult in closing the gaps in this area. This article clarified the importance of narrowing the study's focus using the PICO framework for healthcare: Patient, Negative Pressure Ventilation (NPV), Continuity of Care, and Outcomes (PICO), as presented by (Willis, 2023). A feasible study would involve well-designed clinical trials, comprehensive training for clinicians and providers, a focus on patient benefits, standardized policies and protocols, real-world qualitative case study implementation, assessment of patient outcomes, surveys, satisfaction ratings, and cost analysis for hospital stakeholders.
To determine when it is sufficient, approval from providers and hospital stakeholders is needed to use it for qualitative case satisfaction ratings. While private hospitals in Arizona currently use it, they do not use it as first-line treatment due to limited awareness of how and when to apply it for specific patients. I would like to narrow the focus on a particular disease, but in the case of cardiopulmonary conditions, this is complicated since such diseases typically fall under either obstructive or restrictive respiratory failure. For the purposes of this question, it would be most effective to compare patient outcomes in both categories to determine which yields better results, ultimately aiming to enhance the hospital's patient's outcomes.

The disparity between the potential benefits of non-invasive positive pressure ventilation (NPV) and its limited application in acute care is a significant challenge in contemporary respiratory care practice. This article outlines key concepts for identifying the problem, understanding its causes, and emphasizing the importance of researching and modifying practices from a socioeconomic perspective (Farmer & Farmer, 2021). It suggests that more well-designed clinical studies are necessary to assess the effectiveness of NPV in patients experiencing acute respiratory failure.
Research is needed to identify which patients are most likely to benefit from NPV in acute care settings. Additionally, studies should focus on developing and testing standardized protocols for NPV usage in these facilities. Beyond clinical effectiveness, research must also investigate the real-world implementation of NPV in acute care environments. Future studies should examine factors such as patient comfort, tolerance, communication abilities, and satisfaction with NPV compared to positive-pressure ventilation. Furthermore, it is essential to explore the most effective methods for training healthcare professionals, including respiratory therapists, nurses, and physicians, in the use of NPV.
Overall, this is a difficult dissertation to write. I am willing to pivot and identify areas to focus on with my faculty, chair holders, and community advisors.
