Assignment task:
I need help in making a narrative reflection. I'm doing my consolidation and the patient i was assign to a female 87 yr. old with mild cognitive impairment and Parkinson disease. She was admitted to orthopedic service after fall resulting in a right ankle fracture. Right leg cast dry and intact. During my second week on the floor first thing i did in the morning is to take her vital sign which are the following. T 36.5; BP: 116/68; R 17; PR 86; O2 Sat 93
The patient is concern that her BP is low because she normally has 137/76-80. I'm not sure what to say at that moment .I didn't notice that the oxygen is low at 93%. So upon presenting my v/s to my clinical instructor she asks me to re take the O2 Sat. I ask the patient to take a deep breath, relax and put the HOB on 90 degree, the oxygen taken is normal. Also my instructor asks me if the resident is taking meds for BP which i didn't know if she had maintenance. Please indicate some nursing intervention if vitals are abnormal and nursing education (reference)
