Problem
I have having issues completing a logic model model (in table format) for a practitioner-level intervention based on the problems and needs of a client in residential treatment. one of the gaps/needs identified is that clients need a needs assessment and need a tailored treatment plan for the individual. Also using CRAFT and surveys as a tool. I previously asked a tutor for help. I then sent it to my professor for further clarification. I was told that it is a program model and not practitioner model.. I have attached what I was told is a program model and not practitioner model that I need to understand.
In the table, describe the types of problems, the client needs, the underlying causes of problems and unmet needs. Then describe interventions that would lead to a change in the presenting conditions. Identify the short- and long-term outcomes that you think would represent an improved condition.
