A 72-year-old female is referred to the psychiatric clinic by her primary provider for an in-depth assessment. The patient needs a new PMH-NP. Her previous PNH-NP had moved out of state, and the previous PMH-NP referred the patient to another behavioral clinic, but the patient did not like the clinic staff. The primary provider reports the patient as agitated and having difficulty calming herself, worrisome, hopeless, and despairing. The patient's primary provider reported a history of depression and suicidal ideation when her husband died 22 years ago. Due to the death of her husband, she presented with anhedonia, psychomotor retardation, weight loss, and sleep disturbance. Her symptoms resolved after two years, and a healthcare provider discontinued the medications a year later. She has had no recurrent episodes until now.
