For the career you chose, describe a realistic scenario where you apply the Swiss Cheese Model to a failure, incident, or accident. What constitutes a "failure" will vary greatly in both severity and consequence depending upon your choice of job. Make sure all four levels and their corresponding elements as described on pages 322-323 of your course textbook (Aviation Psychology and Human Factors) are addressed. Be specific in the details of how each is operating and do not use any of the examples from the readings. In your response, address the following points.

A. Describe the details of the failure, given the context of the specific job and aviation environment.

B. Describe how each level of the model represents a preventive barrier and how/why a hole can exist in that barrier.

C. Describe the consequences of the failure.

D. Describe how the error could have been prevented and any safeguards that should be implemented to prevent such errors in the future.

E. Do you think the Swiss Cheese Model is a good tool to use in developing safety systems? In analyzing errors and accidents? Why or Why not?
